Do you perform Certified DOT Medical Exams?  

Yes, we do the medical part of a Commercial Drivers license and issue a medical drivers certificate immediately after passing the exam.   

Are you  Certified by the Department of Transportation to perform these exams?
          Yes
, as of May 21, 2014 all DOT Medical Examiners must be Certified.

Do you perform pre-employment DOT Medical Exams?  

Yes--  whenever form MCSA-5875 is an appropriate examination form.

Do you do pre-employment drug screenings? 

Yes but ONLY when  requested.  Routinely-- this is not part of medical DOT Exam.

Do you do alcohol screenings?    

Yes     Only when requested            

  How much is the DOT Medical Exam?  

Normally $150 but with coupon (see below) you receive  cash discount to $100 for DOT medical exam. Or just say "I found you on the WEB".

What additional tests do you perform? 

We perform the required urine screening test looking for health related issues.  Only when requested -we will provide Drug testing.

Are appointments required?

Yes we strongly recommend pre set appointments, it helps to arrive with an appointment on a day and time when the physician or provider  is available to perform the examination. You don't want to come when a provider is not available to perform the exam!

What are the acceptable forms of payment?

We  accept CASH, pre-approved corporate checks or credit/debit cards--NO personal checks!

Who signs the medical certificate?

The Medical Examiner who performs the medical examination must sign the Medical Certificate.

Do I have to have the drug-screening test?  

No - only if your employer, company, weight of rig or certain interstate travel require it.

Do you check for drugs when you do a routine DOT Medical Exam?

No-  we are required to do a urine screen but that is more about health issues.

Why do you check urine on a routine DOT Medical exam?   

Mandated test by Department of Transportation - we are checking for protein, blood, sugar and specific gravity only.

Can I give a personal check? 

 No - we accept only cash or credit/debit cards.

ABOUT OPTIONAL DRUG TESTING

How soon will I know if I passed my drug test? 

If you are required to have a drug test, results are usually provided within several days of giving a urine sample to either LabCorp or Quest.  This unfortunately is out of our control, once a sample is collected.

Who will notify me if I passed the drug test?

Our office will call you or your employer depending on who is indicated on the response form after you have passed. 

 Who will notify me if I fail the drug test?  

You will be contacted by a physician known as an Medical Review Official (MRO) usually not from our office or even from this area.  He will discuss with you directly the steps to deal with a positive finding.

Will I be given a CDL (commercial Driver License) Medical Exam Card today if I pass?

Yes--  it is known as MEDICAL EXAMINER'S CERTIFICATE

Will I be given a CDL Medical Exam Card  even if I haven't yet provided a urine sample to LabCorp for optional drug testing?

Yes

  What are the Classes of License?

The Federal standard requires States to issue a CDL to drivers according to the following license classifications:

  • Class A -- Any combination of vehicles with a GVWR of 26,001 or more pounds provided the GVWR of the vehicle(s) being towed is in excess of 10,000 pounds.
  • Class B -- Any single vehicle with a GVWR of 26,001 or more pounds, or any such vehicle towing a vehicle not in excess of 10,000 pounds GVWR.
  • Class C -- Any single vehicle, or combination of vehicles, that does not meet the definition of Class A or Class B, but is either designed to transport 16 or more passengers, including the driver, or is placarded for hazardous materials.

Is getting a medical certificate mandatory for all CMV drivers in the United States?

In general, all CMV drivers driving in interstate commerce within the United States must obtain medical certification from a Medical Examiner. CMV drivers from Canada and Mexico can be medically qualified in their countries.


How should I prepare for the test?

Please bring two forms of identification one being photo ID driver's license.  No special preparations are required.  Be mindful of proper diet, exercise,  avoiding alcohol and tobacco use to help achieve a more ideal body weight and blood pressure. 

Does my Health Insurance cover  a DOT Medical Examinations?

NOT usually.  This will depend on your insurance policy.  Our policy is payment at time of service for these examinations. A receipt for this examination is provided.

Is it possible to ONLY have a drug screening test performed?

Yes, simply stop by our office and pick up a LabCorp or Quest Collection Form.    After completion of a simple form at our office and paying the $85 fee, you will be given the collection form and a list of area LabCorp and Quest locations that provide this service.  Call ahead to confirm they are doing DOT urine collection at that location presently.  At your convenience you may go to any conveniently located LabCorp/Quest collection site listed with our form to provide a urine sample.  If you decide to go to another LabCorp/Quest other than we recommend,  make sure to check with that particular  LabCorp  site about their being a DOT urine collection site!!  A few are not DOT urine collection sites!

How long is my Medical Exam Card valid for?

Two years from time of issue is the maximum.  Circumstances may call for a more limited card.

Will you recall me before my DOT Medical Exam Card expires?

Yes as part of the service our office  we will automatically send you a post card reminding you approximately a month before expiration.

Do I need to bring any of the medical forms with me?

No.  The medical forms, Form MCSA-5875, MCSA-5876) Medical Examination Report For Commercial Driver Fitness Determination will be provided by our office.  You can download the form to fill out before your appointment  from the link provided to the FMCSA (Federal Motor Carrier Safety Administration) website.  Our office will also provide the Medical Examiner's Certificate to our qualified drivers after the examination.

If a Urine Drug Screen is not required--- then what do you test for when you take my urine?

We perform the required urine dipstick test to evaluate for protein, blood, glucose (sugar), and specific gravity (concentration of particles in urine).  It helps screen for disease such as diabetes, kidney problems, etc.

What is OSA?
This is Obstructive Sleep Apnea.  Defined briefly as holding your breath repeatedly while sleeping. Indications of OSA require a sleep study to be performed and treatment options to be reviewed
