 Do you perform DOT Medical Exams?  

Yes, we do the medical part of a Commercial Drivers license and issue a medical drivers certificate immediately after passing the exam.

Do you perform pre-employment DOT Medical Exams?

Yes

Do you do pre-employment DRUG SCREENINGS? 

Yes when requested

Do you do alcohol screenings?                     

Yes when requested

How much is the DOT Medical Exam?   $45

Normally $60 but with pre-registration or a printed coupon (see below) you receive a 25% savings to $45 for DOT medical exam.

What additional tests do you perform? 

When requested we do Drug Screen, Alcohol Screen and  TB testing

Are appointments required?

No, but suggested, it helps to arrive on a day and time when the physician  is available to perform the examination.

What are the acceptable forms of payment?

Alliance Rehab accepts cash, corporate checks or credit/debit cards

Who signs the medical certificate?

The Medical Examiner who performs the medical examination must sign the Medical Certificate.

Do I have to have the drug-screening test?  

No - only if your employer, company, weight of rig or interstate travel require it.

Do you check for drugs when you do a routine DOT Medical Exam?

No

Do you check urine on a routine DOT Medical exam?   

Yes - we are checking for protein, blood, sugar and specific gravity only.

Can I give a personal check? 

 No - we accept company checks, cash or credit/debit cards.

How soon will I know if I passed my drug test? 

Usually within several days of providing a sample to LabCorp.

Who will notify me if I passed the drug test?

Our office will call you or your employer depending on who is indicated on the response form after you have passed.  Documentation will be sent to you or your employer.

 Who will notify me if I fail the drug test?  

You will be contacted by a physician known as an Medical Review Official (MRO).  He will discuss with you directly the steps to deal with a positive finding.

Will I be given a CDL (commercial Driver License) Medical Exam card today if I pass?

Yes

Will I be given a CDL Medical Exam card  even if I haven't yet provided a urine sample to LabCorp for optional drug testing? 

Yes

What are the Classes of License?

The Federal standard requires States to issue a CDL to drivers according to the following license classifications:

• Class A -- Any combination of vehicles with a GVWR of 26,001 or more pounds provided the GVWR of the vehicle(s) being towed is in excess of 10,000 pounds.
• Class B -- Any single vehicle with a GVWR of 26,001 or more pounds, or any such vehicle towing a vehicle not in excess of 10,000 pounds GVWR.
• Class C -- Any single vehicle, or combination of vehicles, that does not meet the definition of Class A or Class B, but is either designed to transport 16 or more passengers, including the driver, or is placarded for hazardous materials.

Is getting a medical certificate mandatory for all CMV drivers in the United States?

In general, all CMV drivers driving in interstate commerce within the United States must obtain medical certification from a Medical Examiner. CMV drivers from Canada and Mexico can be medically qualified in their countries.

Can I get a TB test at the same time?

Yes Alliance Rehab -- when requested will perform a TB test although results will have to be checked in approximately 48 hours.  

Does my Health Insurance cover DOT Medical Examinations?

This will depend on your insurance policy.  Our policy is payment at time of service for these examinations. A receipt for this examination is provided

Is it possible to just have a drug screening test performed?

Yes, simply stop by the office and pick up the LabCorp form after completion of 1 simple form and paying the $50 fee.  At your convenience go to any conveniently located LabCorp collection site to provide a urine sample.

How long is my Medical Exam Card valid for?

Two years from time of issue.

Will you recall me before my DOT Medical Exam Card expires?

Yes as part of the service at Alliance Rehab we will automatically send you a post card reminding you approximately a month before expiration.
